چکیده

Introduction The importance of an epicardial substrate for cardiac arrhythmias has been increasingly recognized. Combined endocardial and epicardial mapping and radiofrequency catheter ablation (RFCA) has become the standard strategy for treatment of ventricular tachycardia (VT). The subxiphoid approach is the standard and most frequently used method for percutaneous epicardial access. However, complications of the subxiphoid approach are not uncommon, even in experienced centers, with reported rates of 4% to 7%. In addition, obtaining epicardial access using the subxiphoid approach may be difficult because of extracardiac structures located on the epicardial needle trajectory. Therefore, the epicardial approach to reduce complications and as an alternative to the subxiphoid approach is essential for epicardial RFCA. We described the case of a patient with successful epicardial access twice and RFCA using a parasternal intercostal approach, which is an alternate solution when the standard subxiphoid approach to obtain epicardial access is difficult.

Subxiphoid surgical approach for epicardial catheter-based mapping and ablation in patients with prior cardiac surgery or difficult pericardial access.

BACKGROUND Percutaneous epicardial mapping and ablation are successful in some patients with ventricular epicardial reentry circuits but may be impossible when pericardial adhesions are present, such as from prior cardiac surgery. The purpose of this study was to evaluate the feasibility of direct surgical exposure of the pericardial space to allow catheter epicardial mapping and ablation in th...

Epicardial approach to the ablation of ventricular tachycardia in coronary artery disease: an alternative or ancillary approach.

In this issue of the Journal, Sosa et al. (1) evaluate the feasibility, safety and efficacy of an epicardial approach to ablate ventricular tachycardia secondary to myocardial infarction. Sosa and his colleagues have recently introduced the technique of epicardial mapping to approach and ablate ventricular tachycardia due to Chagas’ disease (2,3). The technique involves introducing a standard a...

Accessory atrioventricular pathways refractory to catheter ablation: role of percutaneous epicardial approach.

BACKGROUND Epicardial mapping and ablation of accessory pathways through a subxiphoid approach can be an alternative when endocardial or epicardial transvenous mapping has failed. METHODS AND RESULTS We reviewed acute and long-term follow-up of 21 patients (14 males) referred for percutaneous epicardial accessory pathway ablation. There was a median of 2 previous failed procedures. All patien...

Epicardial left ventricular mapping using subxiphoid video pericardioscopy.

We report a novel subxiphoid video pericardioscopy approach for epicardial mapping that allows direct visualization of the epicardium with minimal use of fluoroscopy. The FLEXview system (Boston Scientific Cardiac Surgery, Santa Clara, CA), which is capable of a free navigation around the heart owing to its flexible neck, was inserted into the pericardial space through a small subxiphoid incisi...

Pulmonary Embolism as a Complication after radiofrequency catheter ablation Study: A Case Report

Pulmonary thromboembolism (PTE) is a fatal condition that may rarely occur due to complications of coronary catheter insertion. In this case report, a 41-year-old male was presented 48 hours after radiofrequency catheter ablation(RFCA) for the management of Wolf-Parkinson-White syndromewith acute onset of dyspnea, hemoptysis, and chest pain. The physical examination revealed coarse crackles in ...
